HPCG offers in-home hospice care wherever you call home. We serve patients in their private residences, in assisted-living residences, or in long-term-care facilities. HPCG patients and their families are usually in the midst of a difficult journey. We have a team of professionals and volunteers to support the numerous patient and family needs. Our HPCG team includes: • Nurses • Nurse Technicians • Physicians • Medical Social Workers • Volunteers • Chaplains & Grief Counselors • Dieticians • Physical Therapists • Occupational Therapists • Speech Therapists Assisted-living or long-term-care facility-based hospice care: Our HPCG team works closely with facility staff members to serve residents with a life-limiting illness, and who have been referred to HPCG. HPCG also works with the families of facility residents. Wherever an HPCG patient resides, they receive the same compassionate care and support, including: • Expert pain and symptom management • Medicine related to the life-limiting illness • Medical equipment and supplies related to the life-limiting illness • 24 hr/7 days a week, on-call nursing assistance • Spiritual care • Volunteer support • Help with advanced directives (health care power of attorney and living will) • Counseling for HPCG patient and family members • Grief counseling for children and adults • Nursing and social work consultation for out-of-town family members by phone or email
